Synergy Partners, LLC is a Manager of Comprehensive Provider
Network (MCPN). We are a publicly funded behavioral health
organization that provides a comprehensive array of services for adults and
children with developmental disabilities and their families.
Our mission is to operate comprehensive and seamless provider networks on
behalf of persons with developmental disabilities and mental illness, children
in the juvenile justice system and others requiring community-based
services and supports.
Synergy believes that well-informed consumers are
in a better position to self-direct their services and supports. Therefore,
Synergy will work diligently with consumers, families, and independent
facilitators to ensure that the person-centered planning process improves and
renders meaningful results.
One of our primary goals is to provide a comprehensive array of services
to persons with developmental disabilities. We assist consumers and
their families to develop and implement person-centered plans that
support each individual's right to effective and efficient supports, services
and treatment.
